Coordinated Entry Committee

Purpose: Coordinated entry (CE) is a system which prioritizes homeless persons with the most severe service needs for placement in CoC funded housing. This committee reviews the CE system to ensure the system is equitably meeting the community’s needs and HUD requirements.

Activities: Updating policies, procedures and assessment tools, as well as reviewing placements on a semi-annual basis.

CE Lead Agency: CARES of NY, Inc.

Meeting frequency: Once a month, last Thursday of the Month at 9:15AM

Meeting Type (In-Person or Virtual): Currently virtual

Unsheltered Homeless Count Committee

Purpose: Organizes and conducts the annual unsheltered homeless count in the last 10 days of January.
Activities: Coordinates canvassing of known locations, provides education and training about the count, secures donations for outreach activities, and coordinates agency outreach events.

Program Review Committee

Purpose: This committee monitors, reviews and ranks all existing CoC and ESG funded projects against CoC and HUD performance measures, as well as selects any projects for reallocation or new funding.
Activities: Reviews and ranks all renewal and new CoC applications, as well as reviews monitoring letters issued by the CoC administrator.

Committee to End Youth Homelessness

Purpose: Act as the planning and implementation body for the Youth Homeless Demonstration Grant (YHDP), a youth focuses initiative to develop and implement a Coordinated Community Plan (CCP) to prevent and end youth homelessness.

Activities: Collect and analyze cross systems data and obtain input from stakeholders, including youth with lived experience, for the CCP. Oversee CCP implementation, report accomplishments, and recommend annual Plan updates.

Data Committee

Purpose: Analyze data to make recommendations to improve the performance of the county’s homeless delivery system at the program and system level. Primary data source will be the CoC’s Homeless Management Information System (HMIS)

Activities: Review and analyze quarterly HMIS reports, annual performance measures, longitudinal system data, data quality reports as well as other community data to make recommendations for program and system improvements.
